Our team consists of Community Health Manager, Community Health Nurses, Community Health Outreach Worker, Maternal/Child Health Nurse, Registered Dietitians, Tobacco Cessation Coordinator/Administrative Assistant, Transportation Coordinator/Aide, Diabetes Case Manager, Diabetes Prevention Coordinator, Lifestyle Coach, Data Coordinator, WIC Coordinator and Assistant, and Employee Wellness Coordinator. It is our goal to serve our community by providing on-site and outreach services and activities aimed towards prevention, thereby empowering those we serve to take control of their own health and wellness.
