Staff
Our team consists of Community Health Manager, Community Health Nurses, a Maternal/Child Health Nurse, Registered Dietitian, Tobacco Prevention Coordinator, Transportation Coordinator/Aide, Diabetes Case Manager, Prevention Coordinator, Lifestyle Coach, Data Coordinator, and Switchboard Operator/Administrative Assistant. It is our goal to serve our community by providing on-site and outreach services and activities aimed towards prevention, thereby empowering those we serve to take control of their own health and wellness.
